    AFFIDAVIT OF UNDERTAKING
    I, _____________________, of legal age, Filipino citizen, single, a
    resident of Block 246 Lot 10 Arujville Subdivision, Butuan City, after having been
    duly sworn in accordance with the law, do hereby depose and state that:

    1. I shall secure the occupancy permit from appropriate government


    office(s) before December 31, 2019;

    2. I hereby waive all rights and privileges in my business permit if I fail


    to comply the requirements of occupancy permit on or before December 31, 2019;

    3. My business permit shall be revoked/cancelled by the City


    Government of Butuan if I fail with the provisions/conditions of this affidavit of
    undertaking;

    4. I shall observe all the existing laws and ordinances and comply with
    all the regulatory requirements mandated by law for the continued operation of my
    business and may be held criminally, civilly, and/or administratively liable for
    violation of law or ordinance;

    5. I am wholly amenable to comply with the Undertaking otherwise it


    shall result in the imposition of penalty or penalties as provided by law or warrant
    the suspension, cancellation, or revocation of the Mayor’s/Business Permits;

    6. I shall automatically cease and desist from my business operation for


    failure to comply with this undertaking after the December 31, 2019 deadline
    imposed by the City Government of Butuan.

    7. I am executing this Affidavit to attest the truth of the foregoing and


    for whatever legal purpose this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this
