    AFFIDAVIT OF DISCREPANCY
    (Date OF Birth)

    I, ALSHEMAR IDJIRAEL AMIRUL, of legal age, single, Filipino, a resident of


    Stanvac, Lower Calarian, Zamboanga City after having been subscribed to an oath, hereby
    depose and say THAT:

    1. In my Certificate of Live Birth as recorded in the Office of Civil Registrar of the


    Municipality of Sitangkai, Tawi Tawi and in my Official Transcript of Records
    issued by Mahardika Institute of Technology, Inc., it is stated that I was born on
    DECEMBER 22, 1986;

    2. I am a bonafide member of the Philippine National Police and was issued a PNP
    Identification Card but my date of birth recorded and appearing in the said
    identification card is DECEMBER 22, 1996 which is erroneous because my
    true date of birth as stated above is DECEMBER 22, 1986;

    3. I am executing this affidavit to attest the truth of the foregoing facts and to
    explain the discrepancy in my Certificate of Live Birth and my PNP
    Identification Card as regards to my date of birth;

    4. Further, I am declaring that from this date hereon I would be using my true and
    correct date of birth of DECEMBER 22, 1986 in all my papers, records, other
    documents and for any legal purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 15th day of August
    2019 at Zamboanga City, Philippines.
