    AFFIDAVIT

    I, DONALD ARGANA DELOY, of legal age, single, Filipino and a resident of Brgy.
    Rizal, Sogod, Southern Leyte, after having been duly sworn to an oath in accordance with law,
    hereby depose and say:

    l. That I worked as a grinder at Valenzuela City in the year 2004 -2005 and was residing
    at 300 Main road, St. Rasinel Subdivision , Veinte Reales Valenzuela City;

    2. That I applied for a TAX IDENTIFICATIONA NUMBER (TIN) at BIR,Valenzuela


    City, RDO 24;

    3..That as such, I was issued a Tax Identification Number (TIN) 233 136 624 000 by the
    aforementioned Office ;

    4. That at present , I am residing at Brgy. Rizal, Sogod, Southern Leyte ;

    5. That for the above-mentioned reason, I hereby request the BIR to transfer my TIN
    from RDO 24 to RDO 90 as I am now living at Brgy. Rizal, Sogod, Southern Leyte
    which place is under the jurisdiction of RDO 90;

    6. That I executed this affidavit freely and voluntarily in order to attest to the truth and
    veracity of the foregoing and for whatever legal intents and purposes this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 19th day of October, 2011
    in the Municipality of Sogod, Southern Leyte, Philippines.
