Causes
There are several causes of presbycusis including:
Repeated exposure to loud sounds, music, or equipment which can damage the fragile
hair cells within the inner ear involved in hearing
Hereditary or genetic influences
Risk Factors
Presbycusis is more common in men, and in people over 75 years old. Other factors that may
increase your chance of presbycusis include:
ANATOMY
The normal ear can be divided into three anatomic areas: the external ear, middle ear, and
inner ear (figure 1). Disorders of any of these areas can contribute to hearing loss. The
external ear includes the pinna and the external auditory canal, which directs sounds to the
middle ear. The middle ear includes the tympanic membrane, tympanic cavity, ossicles, and
the eustachian tube. The middle ear functions to pass sound vibrations from the external ear
to the inner ear. The inner ear is a geometrically complex, fluid-filled organ that resides
within the dense otic capsule in the temporal bone. The inner ear is the organ of hearing
(cochlea) and balance (vestibular system), both of which translate motion of fluid around hair
cells (from either sound or head acceleration) into neural signals. The stria vascularis, a
system of small blood vessels, produces the fluid (or endolymph) for the scala media, one of
three fluid-filled compartments of the cochlea. The neural signals produced by the hair cells
and surrounding fluid enter the spiral ganglion and are subsequently carried to the brain by
the vestibulocochlear (eighth) cranial nerve.
PATHOPHYSIOLOGY Hearing loss can be subdivided into two broad categories:
conductive and sensorineural. Conductive hearing loss is characterized by the inability to
mechanically transmit sound vibrations from the environment to the inner ear. Conductive
loss is due to disorders of the external and middle ears. Sensorineural hearing loss is
characterized by the inability to effectively transduce sound information into usable neural
signals. The majority of sensorineural loss is the result of disorders of the inner ear itself and
is not directly related to dysfunction of the vestibulocochlear nerve. Thus, the term nerve
deafness is often a misnomer given that the primary dysfunction occurs in the inner ear, not
the nerve. Presbycusis is a true sensorineural loss, in which both cochlear hair cells and, to a
lesser extent, the spiral ganglion cells in the vestibulocochlear nerve can be affected

Temporal bone histology provides some insight to the underlying pathophysiology of
presbycusis. In the histopathology classification system, presbycusis is subdivided based on
the associated audiometric pattern of loss, with abnormalities of inner ear vasculature, hair
cells, and membranes all contributing to audiometric findings [20,22]. The three main types
of presbycusis proposed by this system include:
Neural, characterized by loss of ganglion cells and a variable pattern of hearing loss

With presbycusis, hearing loss is usually very gradual, affecting both ears equally.
Diagnosis
Your doctor will ask about your symptoms and medical history. A physical exam will be
done. The doctor will perform a visual exam of your ear canal and eardrum with a lighted
instrument called an otoscope.

Treatment
Talk with your doctor about the best treatment plan for you. Treatment options include the
following:
Hearing Aids and Assistive Listening Devices
If it is determined that a hearing aid may be useful, the audiologist will conduct several tests
to determine the type of hearing aid that will best improve hearing of speech. The extent of
benefit varies according to the cause and degree of hearing loss. Sometimes hearing aids will
need to be replaced with other models if hearing loss progresses. Some people with
presbycusis may benefit from telephone amplifiers that help hear speech on the telephone.
Cochlear Implants
For certain people with very severe hearing loss that is not improved by a simple hearing aid,
a cochlear implant device may improve sound generation to the brain. It may provide partial
hearing to the profoundly deaf.
Prevention
To help reduce your chance of presbycusis, take the following steps:
Follow treatment plans that help manage health conditions that may cause hearing
loss.
Avoid repeated exposure to loud noises and sounds of any type, including those at
work, home, and during recreation.
When working with loud machinery or in loud environments, wear protective ear
plugs or ear muffs.
If you smoke, talk to your doctor about how you can quit.

ANATOMI - Telinga normal dapat dibagi menjadi tiga bidang anatomi: telinga eksternal,
telinga tengah, dan telinga bagian dalam ( gambar 1 ). Gangguan salah satu bidang ini dapat
berkontribusi untuk gangguan pendengaran. Telinga eksternal meliputi pinna dan saluran
pendengaran eksternal, yang mengarahkan suara ke telinga tengah. Telinga tengah meliputi
gendang telinga, rongga timpani, ossicles, dan tabung eustachius.Fungsi telinga tengah

Pemeriksaan fisik telinga luar harus normal dalam presbikusis. Pemeriksaan otoscopic
mungkin berguna untuk menilai penyebab potensial lain gangguan pendengaran seperti
cerumen impaksi, infeksi, perforasi membran timpani, atau tumor (misalnya, exostosis,
osteoma, polip).
Meskipun Audiometri adalah tes diagnostik yang paling banyak diterima untuk gangguan
pendengaran, tes suara berbisik adalah tes sederhana yang dapat dilakukan di kantor dokter

The Weber dan Rinne tes, menggunakan garpu tala, dapat membantu membedakan gangguan
pendengaran konduktif atau sensorineural, dan mengidentifikasi gangguan pendengaran

Pengobatan

Implan Koklea
Bagi orang-orang tertentu dengan gangguan pendengaran sangat berat yang tidak
ditingkatkan dengan alat bantu dengar sederhana, implan koklea perangkat dapat
meningkatkan generasi suara ke otak. Ini dapat memberikan pendengaran parsial untuk
sangat tuli.
