Perforator Based Propeller Flaps in Limb Reconstructive Surgery: Clinical Application and Literature Review2014 •

Perforator-Pedicled Propeller Flaps for Lower Extremity Reconstruction2020 •
Reconstruction of the lower extremity is considered a major challenge due to frequent bone exposure and the absence of local tissue redundancy, as well as often due to the presence of vascular insufficiency. Many surgeons have preferred free flaps especially for reconstructing the more distal lower limb defects until the evolution of pedicled perforator flaps and propeller flaps in particular provided a like-with-like reconstruction of the lower extremity without affecting the main vessels of the limb or the underlying muscles, and without the risk of any microanastomosis especially in patients with multiple comorbidities. Perforator-pedicled propeller flaps as local flaps in the lower extremity are best suited for small- and medium-sized defects with minimal donor-site morbidity, regardless of the cause of the defect. Any of the three source vessels of the leg can provide reliable perforators for propeller flap coverage of the distal leg and foot. The three main risk factors that a...

Perforator Propeller Flaps for the Coverage of Middle and Distal Leg Soft-tissue Defects2018 •
Local propeller flaps preserve the main vascular arteries of the lower extremity and muscle function, avoiding the need for a microsurgical anastomosis and the benefit of providing a "like with like" coverage. Our goal in this study was to demonstrate the versatility, safety, and complications of the local propeller flaps for lower extremity reconstruction. We present a series of 28 patients in whom we used local propeller flaps to restore small-to-medium soft-tissue defects of the lower limb in different hospitals of Managua, Nicaragua. Flap average dimensions were of 48 cm. Flap rotation was performed in 180 degrees in 85% of the cases. The propeller flaps were based on a single perforator, from the posterior tibial artery in 50%, anterior tibial artery in 39.3%, and peroneal artery in 10.7% of the cases. Complications occurred in 14% of the propeller flaps performed, with 3 partial necrosis of less than 15% of the flap transposed. Complications of the patients occurred ...

A Study of Clinical Applications of Perforator-Based Propeller Flaps in Leg Reconstructive Surgery: A Single-Center Experience of 40 Cases2019 •
Soft tissue management in lower limb poses a considerable challenge to a reconstructive plastic surgeon. Perforator-based propeller flaps can cover large wounds without sacrificing a major vascular axis and allows reconstruction using nearby similar tissues, thereby following the principle of replace like with like. This study was undertaken with the objective to look for the results of propeller flap in lower limb reconstruction and how the results can be improved. This was a retrospective study on 40 patients who underwent the reconstruction of soft tissue defects of lower limb with propeller flap. All the relevant data were collected and analyzed. Patients with soft tissue defect anywhere in the leg were included, but patients who had history of degloving injury, peripheral vascular disease, and diabetes mellitus were excluded. All these patients were divided in 2 groups according to the angle of rotation of propeller flap. Group I had 20 patients in which the flap was rotated by...

Propeller perforator flaps in distal lower leg: evolution and clinical applications2012 •
Simple or complex defects in the lower leg, and especially in its distal third, continue to be a challenging task for reconstructive surgeons. A variety of flaps were used in the attempt to achieve excellence in form and function. After a long evolution of the reconstructive methods, including random pattern flaps, axial pattern flaps, musculocutaneous flaps and fasciocutaneous flaps, the reappraisal of the works of Manchot and Salmon by Taylor and Palmer opened the era of perforator flaps. This era began in 1989, when Koshima and Soeda, and separately Kroll and Rosenfield described the first applications of such flaps. Perforator flaps, whether free or pedicled, gained a high popularity due to their main advantages: decreasing donor-site morbidity and improving aesthetic outcome. The use as local perforator flaps in lower leg was possible due to a better understanding of the cutaneous circulation, leg vascular anatomy, angiosome and perforasome concepts, as well as innovations in f...

The Propeller Flap for Traumatic Distal Lower-Limb Reconstruction2019 •
Background: Defects in the distal third of the leg are difficult to cover and often require free tissue transfer, even for defects of limited sizes. Propeller flaps have been designed specifically as an alternative to free tissue transfer but at times have been associated with unacceptably high complication rates. We therefore aimed to prospectively assess our own institutional experience with this technique and to define its role in lower-limb reconstruction. Methods: All patients who had been managed with reconstruction of the distal part of the leg with a propeller flap between 2014 and 2017 were included in the study. Demographic, clinical, and follow-up data on the patients and surgical procedures were recorded with special focus on the complication profile. Results: Twenty-six patients underwent propeller flap reconstruction of the distal part of the leg: 12 flaps were based on the posterior tibial artery, and 14 were based on the peroneal artery. Postoperative complications d...

Species diversity and catch per unit effort of fish in a perturbed tropical lake, Ibadan, Nigeria2024 •
Eleyele Lake serves the purpose of domestic water supply, transportation, and fisheries; therefore, it is a source of food and livelihood to the artisanal fishers in Ibadan. However, various reports indicated the perturbed state of the lake, which can have adverse effects on the ecosystem services including fisheries. Therefore, this study investigated the catch per unit effort (CPUE) of fish in the Eleyele Lake and some physicochemical parameters in order to establish the status of the fisheries of the lake. Water samples were collected from the lake and analyzed. The fish catches, effective fishing effort (EFF), and CPUE of the lake were determined. The dissolved oxygen and transparency levels were not within the recommended permissible limits. Fish encountered in the lake during the study period belong to nine families and fifteen species. Cichlidae, Polypteridae, and Clariidae were the most frequently encountered families. Shannon-Weiner (2.23), Margalef (2.14) diversity indices, and Evenness (0.87) were highest during the sampling period in 2019. The EFF was 0.37 kW day−1. The mean CPUE increased from 9.55 ± 1.67 kg·kW day−1 in 2019 to 38.07 ± 30.89 kg·kW day−1 in 2021 and declined to 6.44 ± 3.02 kg·kW day−1 in 2023. The forage /carnivore ratio was 0.70 in 2019, 0.35 in 2021, and 54 in 2023 by number indicating an imbalance in the fish population. This study implies that effective management and sustainable exploitation of the fisheries in the Eleyele Lake require improvement of water quality and enforcement of a closed season.

Evangelos Fysikas, "The 7th issue of National Bank of Greece (1890's)", IBNS Journal 63/2, 2024National Bank of Greece (N.B.G.) is the largest and the oldest still operating bank in Greece, with its history being strongly associated with the history of modern Greece. It was established in 1841 and until 1928 had the privilege of issuing banknotes. Its banknotes are sought-after from Greek banknotes collectors and some of them are extremely rare and only a handful of them are extant today. In this article we are describing the short-lived 7th issue of N.B.G. banknotes which is considered relative scarce.
