PurposeAdjuvant chemotherapy for resected non–small-cell lung cancer (NSCLC) is now accepted on the basis of several randomized clinical trials (RCTs) that demonstrated improved survival. Although there is strong evidence that adjuvant chemotherapy is effective in stages II and IIIA NSCLC, its utility in stage IB disease is unclear. This report provides a mature analysis of Cancer and Leukemia Group B (CALGB) 9633, the only RCT designed specifically for stage IB NSCLC.Patients and MethodsWithin 4 to 8 weeks of resection, patients were randomly assigned to adjuvant chemotherapy or observation. Eligible patients had pathologically confirmed T2N0 NSCLC and had undergone lobectomy or pneumonectomy. Chemotherapy consisted of paclitaxel 200 mg/m2intravenously over 3 hours and carboplatin at an area under the curve dose of 6 mg/mL per minute intravenously over 45 to 60 minutes every 3 weeks for four cycles. Physical activity is considered an important and determining factor for the cancer patient's physical well-being and quality of life. However, cancer treatment may disrupt the practice of physical activity, and the prevention of sedentary lifestyles in cancer survivors is imperative. The current study aimed at investigating self-reported physical activity behaviour, exercise motivation and information in cancer patients undergoing chemotherapy. Using a cross-sectional design, 451 patients (18-65 years) completed a questionnaire assessing pre-illness and present physical activity; motivation and information received. Patients reported a significant decline in physical activity from pre-illness to the time in active treatment (p<0.001). Amongst the respondents, 68% answered that they believed exercise to be beneficial; and 78% claimed not exercising as much as desired. Background Breast cancer is the commonest cancer of women the world over, and its incidence is rising, especially in developing countries, where the disease poses a major health care challenge. This growing incidence in developing countries reflects the advanced stage at diagnosis, low levels of public awareness of the risk for the disease, and poor medical infrastructure and expertise, with the resultant poor treatment outcomes. Methods This article provides a collective edited summary of the presentations at the symposium titled “Breast Cancer Care in Developing Countries,” held as part of the Breast Surgery International program at the International Surgical week 2007, Montreal, Canada, August 2007. The aim of the presentations was to bring out the diverse clinical pathological and outcomes-related facts of breast cancer care available to women in several countries. As the incidence of breast cancer continues to rise steadily in the developing world, the lack of awareness of this disease and the absence of breast cancer screening programs make it almost certain that the majority of breast cancers are diagnosed at an advanced stage. In addition, the quality of care available for breast cancer patients varies widely according to where the patient is treated. Results Though there are some centers of excellence providing multimodality protocol-based treatment on a par with the best anywhere in the world, most breast cancer patients receive inadequate and inappropriate treatment because of a lack of high-quality infrastructure—and sometimes skills—and, above all, because of limited financial resources. Conclusions In countries where these limitations are present, there is a need to emphasize public health education, promoting early diagnosis. In addition, resources must be directed toward the creation of more public facilities for cancer treatment. As these goals are met, it is likely that there will be a much-needed improvement in breast cancer care in developing countries.

Withaferin A (WA) is an important withanolide holding promise in cancer treatment and as a relatively safe radiosensitive/chemotherapeutic agent, which is present in traces in all parts of Withania somnifera except the leaves, where as it is reported to be present in only two non-Indian chemotypes (South African chemotype/Israel chemotype 1). The present studies have marked its presence in all Indian populations (wild/cultivated), as well as two identified Indian chemotypes (AGB002 and AGB025). The quantitative dynamics of WA production in Indian populations and interchemotypic hybrids developed at our institute have been studied, and the results were compared with five previously reported chemotypes from Israel, South Africa and India. Renal cell carcinoma (RCC), the most common form of kidney cancer, initially has an asymptomatic clinical course; 25-30% of patients present with metastatic disease at time of diagnosis. Worldwide incidence and mortality rates are rising at a rate of approximately 2-3% per decade. Metastatic RCC (mRCC) is one of the most treatment-resistant malignancies; outcomes are generally poor and median survival after diagnosis is less than one year. Surgery and chemotherapy have limited or no effect, leaving mRCC patients underserved in the realm of cancer treatment. As the world's population ages and the prevalence of risk factors (obesity, hypertension) increases, the burden of mRCC is predicted to increase significantly. With a shift in treatment of mRCC to novel therapies, such as molecularly targeted therapies (MTTs) (e.g., sorafenib and sunitinib), clinicians, payers, and other healthcare decision-makers must re-evaluate the optimal role for new treatments. Timely understanding of the burden of mRCC on individuals and society clearly is needed at this juncture. Using a comprehensive literature review, we assessed the epidemiologic, economic, and health-related quality of life (HRQOL) burdens of mRCC. The annual incidence of mRCC in major European countries, the US, and Japan ranges from 1500 to 8600 cases. However, prevalence data were lacking. The estimated economic burden of mRCC is large; $107-$556 million (2006 USD) in the US and $446 million-$1.6 billion (2006 USD) collectively in select countries worldwide. MTTs have potential to reduce the burden of mRCC and provide substantial value beyond their clinical effectiveness.

This review examines the current literature surrounding the effects of inhibiting specific proteins and enzymes which are essential in the reproduction of cancer cells to eliminate them. The objective was to determine whether inhibition of vital processes within the mitotic stages illustrated any beneficial effect to reduce the replicative ability of cancer cells.

Thus low-level electric current would be an effective treatment of cancer, but this treatment has been languishing for more than two decades due to its lack of patentability.

This review discusses the various aspects of the bio-geochemistry of germanium, and of its technological, economical and environmental importance. Despite the relatively low annual production and consumption of this semi-metal (ca. 80 metric tons/a) there are important technological applications of this element in the semiconductor, infrared optics and fibre optics/telecommunication industries. A small, but not insignificant fraction of this element is used for the production of pharmaceuticals and nutritional supplements, although its actual merits have not been fully demonstrated yet, while they are opposed to chronic toxicity of the element when being administrated at relatively high doses for an extended period of time. Neither the exact mechanism of action in the case of cancer treatment or the treatment of infectious diseases is known, nor the reason for the toxicity of inorganic species of this element. In plants, Ge can partially substitute for B in the case of boron deficiency, although deficiency symptoms are still seen with a lag period of ca. one to three weeks. In biogeochemical respect, germanium and silicon react very similar, as if Ge were a very heavy isotope of Si. Their molar ratio is typically in the order of 0.6 × 10−6, with significant deviations only where germanium is complexed and transported, e.g., by humic-rich waters. Germanium is a very conservative element in biogeochemical terms: It hardly shows involvement in any biogeochemical reaction cycles and is mainly present in the form of complexes or hydroxo-compounds of the tetravalent germanium. The only naturally occurring organogermanium compounds are mono- and dimethylgermanium which are believed to be formed by microbiological activity in continental zones containing Ge-rich minerals, and then are leached into rivers, and finally into the open sea. It becomes evident that, although very sophisticated technological uses of germanium exist, a better understanding of its biogeochemical importance, cycling and reactivity must still be developed.

In this article, Neil B Piller and Jan Douglass report on some studies that have indicated objectively how a specific type of lymphatic massage can help people at risk of and with lymphoedema. They emphasise, however, that massage is but one aspect of the comprehensive and holistic treatment of lymphoedema. Lymphoedema is a multifaceted problem caused by surgery or radiotherapy associated with cancer treatment or associated with a congenital malformation of the lymphatic system. It is often of great complexity, requiring a team approach to its treatment and management. This complexity can mean that existing evidence does not always inform us totally of what is ideal for the current patient and further there is not always an opportunity to select what is ideal due to financial or health professional availability. During the past decade, research and development in the area of synthesis and applications of different nanostructured titanium dioxide have become tremendous. This book briefly describes properties, production, modification and applications of nanostructured titanium dioxide focusing in particular on photocatalytic activity. The physicochemical properties of nanostructured titanium dioxide are highlighted and the links between properties and applications are emphasized. The preparation of TiO2 nanomaterials, including nanoparticles, nanorods, nanowires, nanosheets, nanofibers, and nanotubes are primarily categorized by their preparation method (sol-gel and hydrothermal processes). Examples of early applications of nanostructured titanium dioxide in dye-sensitized solar cells, hydrogen production and storage, sensors, rechargeable batteries, electrocatalysis, self-cleaning and antibacterial surfaces and photocatalytic cancer treatment are reviewed. The review of modifications of TiO2 nanomaterials is mainly focused on the research related to the modifications of the optical properties of TiO2 nanomaterials, since many applications of TiO2 nanomaterials are closely related to their optical properties. Photocatalytic removal of various pollutants using pure TiO2 nanomaterials, TiO2-based nanoclays and non-metal doped nanostructured TiO2 are also discussed.

We describe here the main natural compounds used in cancer therapy and prevention, the historical aspects of their application and pharmacognosy. Two major applications of these compounds are described: as cancer therapeutics and as chemopreventive compounds. Both natural compounds, extracted from plants or animals or produced by microbes (antibiotics), and synthetic compounds, derived from natural prototype structures, are being used. We also focus on the molecular aspects of interactions with their recognized cellular targets, from DNA to microtubules. Some critical aspects of current cancer chemotherapy are also discussed, focusing on genetics and genomics, and the recent revolutionary theory of cancer: aneuploidy as the primum movens of cancer.
