Description
i)切除用内視鏡であって、内腔と、内腔内で可動な少なくとも1つの細長い電極アセンブリと、を含み、該細長い電極アセンブリは電極要素と、吸引内腔が設けられた細長い導電性要素を備える切除用内視鏡を患者体内の外科手術部位に導入することと、
ii)流体を外科手術部位内に導入することと、
iii)電極アセンブリを、電極要素が処置対象の組織に隣接するように操作することと、
iv)電極要素を、外科手術用エネルギー源により起動することと、
v)切除用内視鏡内の内腔を介して、流体が外科手術部位から出て行くことを可能にし、それにより流体および/または組織の残骸を外科手術部位から除去することと、
vi)吸引力を、細長い導電性要素の吸引内腔に供給し、それにより、加熱された流体を電極要素の領域から除去することと、を備える。
なお、本発明の実施態様の参考例として以下のものが想定される。
[参考例1]
少なくともフレームと、前記フレーム上で支持され、第1内腔を構成する鞘と、前記フレームに関して可動な作用要素と、前記作用要素と共に前記鞘内で可動な電極アセンブリと、を含む切除用内視鏡であって、
前記電極アセンブリは少なくとも1つの電極と、電気外科手術用動力を、近位端から前記少なくとも1つの電極に供給するための細長い導電性手段と、を含み、
前記電極アセンブリには、追加的内腔が設けられて、それにより、第1内腔は、前記鞘の前記遠位端における物質の全体的除去が可能であり、一方、前記追加的内腔は、前記電極の極めて近傍から流体の除去が可能であることを特徴とする切除用内視鏡。
[参考例2]
前記細長い導電性手段は中空管内に収納され、前記中空管は、前記追加的内腔を構成していることを特徴とする参考例1に記載の切除用内視鏡。
[参考例3]
前記細長い導電性手段は、それ自体が中空管であり、前記中空管は、前記追加的内腔を構成していることを特徴とする参考例1に記載の切除用内視鏡。
[参考例4]
前記鞘は第1および第2内腔を含み、前記第2内腔は、流体を前記鞘の前記遠位端に供給可能であることを特徴とする参考例1から3のいずれかに記載の切除用内視鏡。
[参考例5]
前記電極アセンブリは、前記電極がぶら下っている2つの細長い導電性アームを備えることを特徴とする参考例1から4のいずれかに記載の切除用内視鏡。
[参考例6]
各前記導電性アームには、追加的内腔が設けられていることを特徴とする参考例5に記載の切除用内視鏡。
[参考例7]
前記追加的内腔を吸引力源に接続するためのコネクタ、および/または前記第1内腔を吸引力源に接続するためのコネクタを含むことを特徴とする参考例1から6のいずれか1項に記載の切除用内視鏡。
Claims (14)
- 切除用内視鏡における使用のための電極アセンブリであって、
縦軸を有する細長いシャフトと、
前記シャフトからぶら下がり、前記シャフトの前記縦軸と角度をなす少なくとも2つのアームであって、アームはそれぞれ近位端と遠位端とを含む、少なくとも2つのアームと、
前記アームを電気外科手術エネルギー源に接続するための第1接続手段と、
前記少なくとも2つのアームの遠位端における組織処置要素であって、該アームの両方からぶらさがる、組織処置要素と、を備え、
前記少なくとも2つのアームの両方は近位端から遠位端に延伸する吸引内腔を含み、前記吸引内腔の前記遠位端は、前記組織処置要素の該少なくとも2つのアームとの接続領域において終端し、前記吸引内腔の前記近位端は、前記吸引内腔を吸引力源に接続するための第2接続手段を含んでいる、
ことを特徴とする電極アセンブリ。 - 前記少なくとも2つのアームのうちの少なくとも1つのアームは、前記アームを電気外科手術用エネルギー源に接続するための前記第1接続手段を構成するワイヤと、前記吸引内腔を構成する中空管と、を含むことを特徴とする請求項1に記載の電極アセンブリ。
- 前記中空管は前記ワイヤを取り囲んでいることを特徴とする請求項2に記載の電極アセンブリ。
- 前記ワイヤは、前記組織処置要素用の構造的支持体を構成する剛性ワイヤであることを特徴とする請求項2または3に記載の電極アセンブリ。
- 前記少なくとも2つのアームのうちの少なくとも1つのアームは、前記吸引内腔と、前記アームを電気外科手術用エネルギー源に接続するための前記第1接続手段と、の両者を構成する中空管を含むことを特徴とする請求項1に記載の電極アセンブリ。
- 前記中空管は、前記組織処置要素用の構造的支持体を構成することを特徴とする請求項5に記載の電極アセンブリ。
- 前記組織処置要素は、カッティングループ、ローラー電極、スライダ電極、またはボタン電極のいずれかを備えることを特徴とする請求項1から6のいずれか1項に記載の電極アセンブリ。
- 前記組織処置要素は、前記吸引内腔と連通している少なくとも1つの開口を含むことを特徴とする請求項1から7のいずれか1項に記載の電極アセンブリ。
- 前記組織処置要素は、前記吸引内腔と連通している複数の開口を含むことを特徴とする請求項8に記載の電極アセンブリ。
- 前記少なくとも2つのアームのうちの少なくとも1つのアームは、前記吸引内腔と連通している少なくとも1つの開口を含むことを特徴とする請求項1から9のいずれか1項に記載の電極アセンブリ。
- 前記少なくとも2つのアームのうちの少なくとも1つのアームは、前記組織処置要素の前記接続領域における物質を、前記吸引内腔内に向けるように構成されている吸引フードを含むことを特徴とする請求項1から10のいずれか1項に記載の電極アセンブリ。
- 前記少なくとも2つのアームは、前記シャフトの前記縦軸の方向に略直交する方向において、組織処置表面を提供するための前記組織処置要素の適切なサイズの表面が設けられるような角度で、前記シャフトからぶら下がっていることを特徴とする請求項1から11のいずれか1項に記載の電極アセンブリ。
- 前記少なくとも2つのアームは、前記シャフトの前記縦軸に対して少なくとも30度の角度で前記シャフトからぶら下がっていることを特徴とする請求項1から12のいずれか1項に記載の電極アセンブリ。
- 前記少なくとも2つのアームは、前記シャフトの前記縦軸に対して少なくとも45度の角度で前記シャフトからぶら下がっていることを特徴とする請求項13に記載の電極アセンブリ。
