Concerns on Generalizability

We read the study by Chung et al [1], which evaluated the effectiveness of a mobile health (m Health) app based on traditional Chinese medicine (TCM) in patients with prediabetes. This randomized controlled trial showed that the TCM m Health app improved physical energy, fitness, and quality of life in patients with prediabetes. However, the small sample sizes, short follow-up time, and multiple comparisons might limit the generalizability of the findings.

In this study, we assess the impact of engagement with a digital behavioral change app (Gro Health) that is certified by the National Health Service (NHS) on the self-reported health outcomes among people diagnosed with T2 D or prediabetes. The EQ-5 D questionnaire was used to capture outcomes as it is a widely recognized and validated tool.

Evaluation of a Web Platform to Record Lifestyle Habits in Subjects at Risk of Developing Type 2 Diabetes in a Middle-Income Population: Prospective Interventional Study

Target populations for prevention comprise people at high risk, such as those in the prediabetes state. Prediabetes is characterized by increased glucose concentrations without reaching T2 D levels [4]. The most effective therapy to decrease T2 D risk is lifestyle modification [5] and, in some cases, addition of pharmaceutical therapy [6]. Weight loss and improvements in body composition have been established as therapeutic targets in prediabetes management [6].

In the United States, 84.1 million people are living with prediabetes, and 2 million people are diagnosed with diabetes annually [1]. According to the World Health Organization, an estimated 422 million people worldwide had diabetes in 2014, and the prevalence continues to rise [2]. By 2050, the Centers for Disease Control and Prevention (CDC) estimates that 1 in every 3 people globally will have diabetes [3].

The COVID-19 pandemic created unprecedented challenges for people with T2 D and prediabetes to access in-person health care and self-care support in the United Kingdom [4,5].

In the meantime, the number of patients with prediabetes was approximately 1.7 million in 2019 in Saudi Arabia with a prevalence of 39% [4]. Prediabetes refers to a person who a higher than normal blood sugar level that is not high enough to be considered diabetic yet. However, without lifestyle changes, individuals with prediabetes are more likely to develop the disease. This indicates that prediabetic patients are at relatively high risk for developing diabetes in the future [6].

Type 2 diabetes mellitus (T2 DM) is a preventable disease; however, most of the 84 million adults in the United States who have prediabetes do not participate in evidence-based prevention programs [1-3]. Although the Diabetes Prevention Program (DPP) study found that people with prediabetes can reduce their risk of developing T2 DM by 58% through participation in an intensive lifestyle modification program [1], personal and logistical barriers limit participation.
