Th... more Transplantation of adipose tissue is used for cosmetic as well as for reconstructive purposes. The methods of harvesting, processing, and transplantation vary in practice. Grafts are harvested almost exclusively using special extraction needles. Smaller fat particles are harvested using fine needles with small diameters of lateral openings and again transplanted using thin injection needles. There is a proven relationship between the diameter of the fat particles and that of the perforations in the extraction needle. The adipose tissue can be harvested from different regions of the body, mostly from the buttocks or the trochanter region and also possibly from the inner sides of the upper thigh or the knee region. Just a few hours postoperatively, the beginning of the neovascularization becomes evident. A certain loss in volume is generally experienced. Finally, the risks and complications of transplantation of autologous adipose tissue are discussed.

There is a high demand for large volume flaps for the reconstruction of congenital or acquired so... more There is a high demand for large volume flaps for the reconstruction of congenital or acquired soft tissue volume defects. The supply of tissue engineered three-dimensional vascularised adipose tissue, in particular for breast reconstruction, is currently limited by the insufficient vascularisation of newly generated tissue. Therefore this study aimed at the de-novo generation of large amounts of vascularised, stable and transplantable adipose tissue by using an adipofascial tissue flap and a matrix scaffold in a growth chamber (1.7 ml) in the rat. The flap was 4% of the overall chamber volume at insertion. Subsequently, we used the method in the pig with a growth chamber of 78 ml volume and performed a pedicled transplantation of the new generated adipose tissue to simulate in-situ breast reconstruction or remote flap generation. Tissue growth, vascularisation and vessel patency within the chamber was non-invasively monitored by Magnetic Resonance Angiography (MRA). Histological sections at harvest showed that the MRA could distinguish between the new tissue in the chamber and the matrix scaffold. At 12 weeks post-insertion, up to 55% of the chamber volume consisted of adipose tissue, which exhibited true hyperplasia without any hypertrophic changes. When transferred to an other site of the body, the new generated adipose tissue stayed stable in volume and showed no fibrotic changes after 22 weeks. Thus, the study demonstrates a promising method to produce significant amounts of vascularised, stable and transferable adipose tissue that can be monitored non-invasively by MRA. This is an important step towards development of a permanent autologous soft tissue replacement and a clinical application.

The extent to which these changes involve structural alterations in membrane lipids has been investigated in CHO cells. Sensitive (AUXB1) and resistant (CH(R)C5) chinese hamster ovary cells (CHO) were grown in culture. Incubations were carried out with R-verapamil (0-10 microM) or the membrane perturbing agents tauro-cheno-deoxycholate (0-1.6 mM, TCDC) and tauro-urso-deoxycholate (0-3.5mM, TUDC). Cell membrane fluidity was determined by electron-paramagnetic resonance spectroscopy and membrane lipids by HPLC and TLC. The resistant CH(R)C5 subline had a higher cell membrane order (lower fluidity, S = 0.7234) in the interface region of the cell membrane than sensitive AUXB1 cells (S = 0.6984) determined using EPR. The MDR-modulator R-verapamil and TCDC, but not TUDC, lowered cell membrane order in a concentration-dependent manner and increased membrane fluidity of the resistant CH(R)C5 subline. TCDC and R-verapamil were without effect on the cell membrane fluidity of AUXB1 cells. These changes were accompanied by alterations in the fatty acid composition of the plasma membrane. Untreated sensitive AUXB1 cells had higher levels of unsaturated fatty acids than resistant CH(R)C5 cells. In CH(R)C5 cells, R-verapamil increased the content of poly-unsaturated fatty acids and TCDC, but not TUDC, increased the content of mono-unsaturated fatty acids. The results demonstrate that resistance modifiers such as verapamil may influence cytostatic drug action by producing structural changes to lipid domains in the plasma membrane.

The mechanism of resistance development is unclear and the expression of P-170 glycoprotein is generally low. The aim of this study, carried out in vitro in sensitive and cisplatin-resistant cell lines, was to examine sensitivity modulation using R-verapamil and cell membrane perturbing agents. Cell growth rates and changes in the order of the cell membrane, determined using electron-paramagnetic resonance spectrometry, were recorded. R-verapamil increased the toxic effect of doxorubicin in the cisplatin-resistant cell line which showed the highest membrane order. Linolenic acid had a similar effect and also increased sensitivity to cisplatin and methotrexate. Bile salts (tauro-cheno-deoxycholate,TCDC, and tauro-urso-deoxycholate TUDC), had little effect on cytotoxicity. These results indicate that R-verapamil and linolenic acid can act as sensitivity modulators in bladder carcinoma cells and that the action of these agents may involve membrane fluidity changes, a phenomenon noted previously in regard to sensitivity modulation in chinese hamster ovary cell lines.

Compromised wound healing can delay adjuvant therapies and limit the patient's prognosis. With current treatment, integrity of the abdominal wall may be maintained. The defect geometry is unique and requires extensive volume for reconstruction. This study describes the surgical technique and reports clinical outcomes of fasciocutaneous posterior thigh flaps as preliminary data for this indication. Thirteen posterior thigh flaps were used in eleven patients between 2013-2015. Patients were prospectively followed-up for the pursposes of this case series. Flap dimensions and volume were measured in two representative cases. The surgical technique is described in detail. Dead space occlusion was achieved in 100% of cases.
