Gigantic breasts are unnaturally large breasts, even by anime standards; they are true gargantuan monstrosities, either because of sheer size or through the combination of size and exceedingly unnatural shape. Essentially, breasts that take at least as much space as the rest of the chest or the abdomen, or that are almost as big but have an absurdly unnatural shape, are gigantic; smaller breasts are the more common huge breasts, even if they're as big as the character's head.
