James William Cusack was the son of Athanasius Cusack and Mary Ann Rotheram.2 He married, firstly, Elizabeth Frances Bernard, daughter of Joseph Bernard, in April 1818.1 He married, secondly, Frances Radcliffe, daughter of Reverend S. Radcliffe, on 10 September 1838.2 He died on 25 September 1861.2
He was Professor of Surgery at Trinity College, Dublin University, Dublin, County Dublin, IrelandG.1 He graduated with a Doctor of Medicine (M.D.)1 He was Surgeon-in-Ordinary to HM Queen Victoria in Ireland.1 He lived at Abbeville, County Dublin, IrelandG.1 He lived at Cussington, County Meath, IrelandG.1
